Todays aging population means theres a growing demand for health care professionals in postacute care and a wave of career opportunities available at Avalon Health Care Shift Monday Friday Full Benefits package This position requires creation of disposition pland and paperwork and social work.
This candidate must ahve a valid SSW license.

The Social Services Director will be responsible to identify and provide for each residents social emotional and psychological needs and the continuing development of the residents full potential during hisher stay at the facility and to assist in the planning for hisher discharge.
Two years experience in social work is required.
Health care experience especially in long term care is strongly preferred.
Bachelors Degree in Social Work.
Masters Degree in Social Work where required.
Basic understanding of computer technology.
Must have ability to read write and follow oral and written directions in English.
Must be able to relate favorably to residents and families and to work cooperatively with other employees and personnel of other community agencies.
Must have knowledge of State and Federal guidelines.
Must have demonstrated skills in communication documentation assessment and social work methods techniques.

Estimated Salary: $20 to $28 per hour based on qualifications.
